- We offer scheduled appointments for evaluations and clearance for participation in sports
- Your child should be up to date on well visits, otherwise we may ask to address the sports physical at a well visit
- We ask that families bring any required forms for their activity with the parent/family portion completed prior to arrival
- There may be a fee for the sports physical as it is not covered by insurance
